We support people with disabilities who want to transition from a nursing facility to a more independent setting in the community. If you want to get out of a hospital or nursing facility and into a home in the community, we can help!
You must have active Medicaid and contact us at least seven days prior to discharge to be eligible.

- Assistive modifications
